Sign Up / Registration / Certification Information

In order to be certified to play during the 2010 season, your child must complete the following requirements. Registration forms are available on this this website and at all sign-ups. See our Calandar of Events section for registration times and dates.

  1. Turn in a completed Contract.   Please note we need a physical address on the contract not a P.O. box.
  2. Birth Certificate. We must see the ORIGINAL and we must keep 2 copies for our records. The only acceptable substitutes (for unusual circumstances) are a Verification of Live Birth (L.A. County is the only jurisdiction that provides this), a valid Passport, or a valid Military ID. PLEASE NOTE WE DO NOT ACCEPT Hospital Certificates. If you do not have your original birth certificate, you may obtain a copy at ine of the following locations:

    NOTE: If your child played last year you must still turn in two copies of the Birth Certificate.

  3. Physical Exam. No child may participate in any activities until he or she has passed a physical medical exam. Please note that most doctors get very busy at this time of year as all children active in fall sports have the same requirement, and you may have a difficult time scheduling an appointment.
    We only accept the ORINGINAL Physical Form NO COPIES, OR FAXES, will be accepted. Ensure that the complete date of the physical exam is included on the form, ie 2/4/10, not 2/04 or 2/10. Please double-check your child's Physical Form as it will be returned to you for completion and delay your child's registration.
  4. Final Report Card (Last Report Card of the 2010 school year). Final report card for the school year showing grades for all quarters.
  5. Completed Parent Code of Conduct form.
  6. Completed Mandatory Parent Volunteer Participation Agreement and $50.00 deposit. Available at sign-ups, on line or parents meeting.
  7. Payment of all Football Fees. ($330.00) This fee covers use of game pants, home and away game jersey's (to be returned at the end of the season), rental of helmets and shoulder pads and trophies. This also covers all association fees for SMYF&C and PCC (Pacific Coast Conference).
  8. Payment of all Cheer Fees. ($485.00) This fee covers cheerleaders skirt, shell, underskirt briefs, and trophies This also covers association fees for both SMYF&C and PCC (Pacific Coast Conference).
  9. Make Weight(football players only) for the desired division. (See age and weight chart.)
  10. Two copies of your current complete Utility Bill (electric, water, or gas) ONLY (no tear offs) NO telephone, cable, etc. The name on the Utility Bill MUST MATCH the parent information listed on the contract. ie. same name and address
